Submit a Test
Once you have completed and answered the
course questions on the provided answer sheet simply follow one of these steps;
On the EZ Answer Sheet: (Choose this option if you currently use an email application such as Microsoft Outlook Express, Microsoft Outlook, Eudora or Mail.)
Enter your name, a credential type from the dropdown, credential number and click on the 'SUBMIT' button located at the page bottom.
E-mail the answer sheet: Choose this option if you currently use an Internet email service such as Yahoo, Gmail or Hotmail etc. You will then need to save your form and return it manually to: (Fill in name of course)@badgerstatelearning.com Include your name, credential type and number.
US mail to: Badger State Learning, Course: (Fill in name of course), 115 Klompen Court, Neenah, WI 54956 Include your name, credential type and number.
